To facilitate a smooth empanelment process, healthcare institutions are generally required to submit the following documents:
- Hospital Registration Certificate
- Clinical Establishment Registration Certificate (if applicable)
- PAN Card of the Hospital/Organization
- GST Registration Certificate
- Cancelled Cheque or Bank Account Details
- NABH/NABL Accreditation Certificate (if available)
- Fire Safety Certificate
- Biomedical Waste Management Authorization
- List of Doctors with Qualifications and Registrations
- Medical Council Registration Certificates of Consultants
- Infrastructure and Equipment Details
- Hospital Brochure/Profile
- Current Tariff and Package Rates
- Memorandum of Association/Trust Deed/Company Incorporation Documents
- Recent Passport-Size Photographs of Authorized Signatories
- Authorized Signatory ID Proof and Address Proof
- Completed TPA Empanelment Application Form
- Existing TPA/Insurance Empanelment Details (if any)
- Any Additional Documents Requested by the TPA or Insurance Company
